G. Checklist:

The checklist, the simplest of the devices, is a prepared list of behaviour or items. The presence or absence of the behaviour may be indicated by checking yes or no, or the type or number of items may be indicated by inserting appropriate word or number.

H. Rating Scale:

The rating scale involves qualitative description of a limited number of aspects of a thing or of traits of a person. The classifications may be set up in five to seven categories in such terms as:
